Out-toeing Q&A
What is out-toeing?
Out-toeing is a condition where the feet point outward while walking or running. It may occur while children learn to walk and can involve the hip (femoral retroversion), thigh bone, or the lower leg (external tibial torsion). Though not always painful, evaluation by a pediatric foot and ankle specialist can correct an irregular gait and optimize the way your child walks, runs, and jumps.
What are the symptoms of out-toeing?
Common signs include:
- Toes pointing outward while standing, walking, or running
- Both feet turned outward
- Awkward walking or running style
- Abnormal gait and frequent tripping or falling
- Leg, knee, or foot fatigue with activity
How is out-toeing diagnosed?
The Mill Creek Foot & Ankle Clinic team reviews your child’s symptoms, activity level, and medical history, examines the feet and ankles, and watches gait while your child walks, jumps, or runs. Your provider may also order X-rays, an MRI, or another imaging test to detect or rule out additional concerns before personalizing a treatment plan.
How is out-toeing treated?
Out-toeing often improves on its own over time with observation. If it hasn’t by age 10—or if it affects function—the team may recommend one or more of the following:
- Physical therapy and targeted stretching/strengthening
- Specialized shoes or orthotics
- Bracing or splinting
- Custom 3D orthotics
- Activity modification and gait training
- Surgery (rare; considered only in severe, persistent cases)
The team follows up periodically to ensure an effective outcome. Mill Creek Foot & Ankle Clinic also treats in-toeing (feet turned inward), which is more common than out-toeing.
